It depends on how far you want the quilt to drop down the sides/bottom of the bed, and how much of that drop is made up of border. I'm going to assume you want to have the squares cover the mattress top, and you'll use borders to create the side and bottom drop. It also depends on if you're talking about 4" finished (sewn) square or 4" cut (3 1/2" sewn) squares.

A standard US twin size mattress is 39" x 75". If you want 4" finished squares (cut at 4 1/2"), you'll need 10 across and 19 down, or 190 squares. if you want 4" cut squares you'll need 11 across and 22 down, or 242 squares.

A standard US full size mattress is 54" x 75". If you want 4" finished squares (cut at 4 1/2"), you'll need 14 across and 19 down, or 266 squares. if you want 4" cut squares you'll need 16 across and 22 down, or 352 squares.

You really should measure your mattress in case it isn't a standard size. If you want to have the squares make up part or all of the drop, measure from the bottom of the drop on one side to the bottom of the drop on the other side for your width. Measure from the top edge of your mattress to the bottom of the drop at the foot for your length. Once you have your width and your length, divide each measurement by the finished sewn) square size. That will tell you how many square you need across, and how many down. Multiply the across number by the down number and you have your total squares.
