depends completely on the organisation - these are not legally defined titles or controlled by some trade body and each organisation defines its own usage.
Team Leader may be an assignment name rather than a job title eg an associate professor may also be a department head simultaneously

Yes! due to the fact that it takes some business's human resource department to make those changes in your record. If it is not change with in a week or two of your promoted I call the human resource office ask why.

Yes. Odd as it may sound but it happens. Not fair though. Usually because they don't want to reflect the promotion in the paycheck.
