It happens when someone wears pants that are too tight in the crotch and the center seam of the pants define and separate the labia so that the shape of them are visible, hence taking the shape of a camel's hoof. Here is a link to a camel's hoof, not a "camel toe" as this would be lewd:

A camel toe that is found anywhere but on the end of a camel's leg is perhaps the most disgusting thing known to mankind ever in the world. I cannot tell you how much a camel toe makes me want to vomit. In fact, I am fighting back vomiting right now just thinking about it. Ladies, please take note here if you have just a second: Spandex is a privilege, not a right.

I downloaded a picture of one once and sure enough; it's just the toe of a camel. (Pretty hairy toe, too.)
