How are the Real Flame Fireplaces?

Well, of course you need ventilation. They work well if the wood is well seasoned and they are built right and the flue is not clogged. Some people love the smell of a burning wood fireplace. And some wouldn't have any other kind. You must be more attentive to the fire than you would a gas fire and use common sense re screens and sparks.

There are a whole lot of 'ifs' to to this question, however if you are planning to put a fire place in your home for heating , it is best to go with a Franklin stove, it is more efficient at heating than open flame. Also fireplaces increase insurance cost and most municipalities are now passing legislation to ban wood burning stoves, so gas may be a better alternative. burning wood will "smell" and an outside air vent is ideal, so as not to pull any already warmed air from in the house up your chimney.
