No way, don't ask her directly if she's a lesbian, that would ruin all your chances. First, ask her for lunch. Since you work at the same place, asking her out for lunch seems pretty "normal" and doesn't look like you're interested in her that way. Then, once you're together, you'll be able to see if she's or not a lesbian, check out her manners, her conversation, etc. If everything goes well at lunch, then you could ask her out for dinner, a movie, or drinks. If she looks at you weird, tell her that more people will go. Anyway, that's what I'd do.

Ask her to lunch... or invite her to some sort of "event".... some daytime thing... that way you could hand her a flyer and not have to say a word.
Don't depend on the fact that she's butch to mean she will make a move. My GF is butch, and incredibly shy.
If it had been up to her, we wouldn't be together... she had an insane crush on me, but I still had to be the one to make the first move (inviting her out)... and I did it in a "just friends" kinda way and then let things go from there.

As a butch myself, i'm gonna say be subtle. Since you work in the same hospital, see if she wants to go to lunch with you, then steer the conversation towards relationships. Let her know your interested. If she is shy and doesnt tell you want you want to know then be a little more straight forward. If she is butch and interested, she will take it from there.

Dont ask her if she is a lesbian. That would offend anyone. Just handle her as if she wasn't. Flirt then get to know her. Once you know her name and a little bit about her, ask her if she wants to go for a drink with you after work. Once you go out, start talking about old girl friends that didn't work out. Then ask her about any relationships she had that didn't work out. If she talks about a man, your good to go:)

Honesty is so under rated. We get so caught up in not wanting to offend someone so we decide to manipulate the situation instead of just coming straigh out and asking, "Are you a lesbian and how about coffee, tea or me". Just be honest.
