Glucose is absorbed directly into the bloodstream, mostly from the small intestine after you've eaten a meal. Your body can also make glucose from fats and proteins when it needs to. It travels in solution with blood plasma, meaning that it isn't 'carried' by a protein the same way oxygen is by hemoglobin. However, to get across the plasma membrane of a cell, a glucose transporter is usually required; you can think of these as enzymes that are bound in the plasma membranes of individual cells that are designed to move glucose out of the blood and into the cell. Once in the cell, glucose may be modified (phosphorylated) by a hexokinase (a type of enzyme) that will prevent it from leaving again.

Glucose is a monosaccharide, it is soluble in water, therefore soluble in blood. This means it literally just dissolves after it is absorbed into the blood stream.
Getting it into the blood in the first place isn’t quite as simple though. This is done using a co transport system which involves the transport of the glucose molecule across the gut bound to a salt molecule (NaCl)
This is the reason it’s important to control your salt levels as well as sugar levels. High salt in your diet would increase glucose uptake which can lead to class 2 diabetes (insulin resistance) due to your constant higher levels of glucose.

Blood is composed of blood cells (red, white), platelets, and other cells; within a water/saline solution called serum. When the glucose from food is absorbed, the glucose molecules are carried within the serum (similar to mixing sugar and water). The cells take the oxygen from the red blood cells, and the glucose from the serum to burn to make energy. The serum also carries other chemicals and molecules, such as sodium, potasium, magnesium, chloride, hormones, medicines, and other molecules.
