If you mean dsturbing as in just a really scary horror that gave me nightmares many listed here are good (bad) ones. Hostel, Hills Have Eyes, House of 1000 Corpses, etc. Then there's a movie called "In The Mouth of Madness" with Sam Neill. That scared the poop out of me. And Event Horizon always gives me chills up my spine (again with Neill). Finally there's a French film called High Tension (or Haute Tension) which wasn't so much disturbing as one of the better horror movies of recent time.
As far as dead-set disturbing a movie called "Twentynine Palms" was billed as a horror movie but was more of a drama with some very disturbing sexual/violent tones toward the end of the movie. I honestly wouldn't recommend it as a good movie, but if you want to see something genuinely disturbing that's your best bet.

I would have to say Blair Witch Project...that was genuine terror. I for one thought it was a true story. Even bought the book w/ copies of the documents. I wanted to take a vacation there (Virginia, I think it was) to investigate for myself. The guys behind hyped it up so well!
Night of the Living Dead, the old, original B&W version. All those brain eating zombies were nightmarish.
The very 1st Nightmare on Elm Street (I think it was 1985). The 1st time I saw it I was jumping at everything.
Child's Play - everytime I see a red headed toddler I freak!!!
Silence of the Lambs - just the thoughts of him eating flesh & that mask were creepy!
The 1st Scream movie - you never knew what to expect.

Hostel was really messed up. I love horror movies and have seen just about all of them and the only one that truly disturbed me was "The Hills Have Eyes" (the new one). I think they pushed the gore-factor a little far. I mean two sisters were raped by freaks while one of their babies was held at gun point, their mom was shot in front of them, the dad was burned alive in front of them, their dogs were slaughtered, and that's just what was carved in my mind. I actually felt sick after I made it through.

I would have to go with "Napoleon Dynamite." My reasoning is simple... how could such utter crap become such a huge hit?! I didn't find it the least bit amusing and find it insulting of my generation. Its unamusing at best, mindless droll at worst. It wasn't scary or disgusting. Just the fact that it was one-upped by so many other, DECENT movies is mindboggling disturbing!

I say Pulp Fiction because I fainted when they stuck a needle in Uma Thurman's heart when she overdosed on drugs.

Disturbing as in "disgust" or just plain disturbing? If it is disturbing, it had to be the "Passion of Christ". Just to see this Godly man treated like an animal, shows you what humanity is like, but yet, Christ wanted to die for those that hated him, and killed him. That is most disturbing because you are ashamed of what man had become and still is, and yet, God sent Christ to forgive us of our sins. Beating Him, knifing Him, spitting on Him, torturing Him, killing Him, seeing Him suffer, for nothing He had done. It made your stomach turn to see the inhumanity of man, and it didn't stop there, it still goes on.
