Biaxin regular is a regular strength medication, which means you will probably have to take it once or twice a day depending on what your doctor has prescribed. Biaxin XL means it is a long acting medication which releases over a longer period so you may have to take it for a shorter time or only once a day.

XL usually means it has been formulated to release over a longer amount of time - i.e. it dissolves more slowly in your stomach so that you don't get the medicine all at once, but get a little constantly throughout the day.
