With respect to #5, I am looking for the '70s
quartet, rightly considered a "supergroup", with whom he and the drummer, while not actual members, got their names credited
on the band's first LP as a quartet.

5. George Reeves-as Cordovan. He also did work and collaborated with reggae artist, country, punk, r&b and other types of artist.
