Many years ago there was a dam in West Virginia that collapsed and flooded part of a small town. I was part of the search and rescue team and I can still remember seeing a hand sticking out of the mud with a red tag (put on by a previous searcher).
Many people died then and I'll never forget that one scene. I don't know that this was the worst I've ever seen, but it was the most memorable. Even watching the 9/11 tower collapse was less visceral than this image. And the 9/11 disaster was pretty bad!

Near the intersection of LOOP 12 and HWY 183 in Dallas, right in front of the Cowboys stadium, a car in the opposite lane drove up the divider wall and flipped over and was hit by several other cars. I was going in the opposite direction but I saw the bottom of the car as it was going up and over. This happened about 15 years ago and I still recall the sight and the sound vividly.
Peace.
EDIT: People have seen and been party to some very horrific things. I am so sorry for you. It felt wrong, somehow, to give you thumbs up, but I did give a lot of thumbs up just to say I'm really sorry you had to see that.
Peace.

The worst thing I have ever seen was seeing my grandmother deteriorate. Before she got sick, she was so healthy and alive. She ate healthy and walked a lot. She would walk to the store, to Wal-Mart or almost anywhere that wasn't too far. Then, she broke her back. A few years later, she found out she was diabetic. A couple more years, they found a tumor in her cervix. She went through radiation and chemotherapy and got through that first battle. Then, they found a mass in her lung. After that, the person we used to see working in the garden, cooking and feeding people that came over to the house, could no longer feed herself. Her death came as a shock to the whole family. She raised me and my brother. To us, she was more than just our grandmother. Seeing her go from healthy to no longer around was the most and still is the worst thing I've ever seen. She was so young to go so early.

When I worked at Willow Run Airport in Michigan I watched a man commit suicide by diving his plane straight down from 15,000 feet. I was the first one out to the wreckage & that sight has stayed with me ever since. What made it infinitely worse to me was that he apparantly changed his mind at the last moment and tried to level out but the g-force just shredded his plane.

A crewmember killed in action, he was about two feet from me when he took a round through the chest, then another hit him in the right eye, that made a "pop" sound I will never forget. Three weeks later, I took a round in the upper thigh area, it made a smacking sound when it hit, then began to burn. The sound of bones snapping always makes my gut knot up.

As a teenager I saw a man 'necklaced' by the ANC (the SA Govt now) as he was suspected to be a police informer. A group of drunk ANC types dragged him out of a car, soaked him in petrol & tied a car tyre around his neck before setting fire to him. Another local tried to shoot the man to stop his pain I guess, but he was beaten up by the crowd. We were rushed away by our Auntie. I was told by my Auntie that the man wasn't an informer, but he was the husband of a woman that the local ANC committe secretary wanted.

A man dropping dead in a pub next to me.
But, hey, the bad guy ex-con we had in our group proved himself the hero of the hour by giving this old man CPR and mouth to mouth. Sometimes Hollywood's right and disaster does bring out the good in people.

In Nigeria as a kid, saw dead bodies lying in the middle of the road, with cars driving on by without stopping to move them (no one wanted to stop and run the risk of being hijacked).
