I had mine tied, burned shut and sections removed about 8 years ago. I better NEVER get pregnant again!
To answer the 2nd part of your question, I've also never known anyone who got pregnant again after a tubal. I guess all my friends had it done right.

As a Mother-Infant nurse, I can tell you I've taken care of my share of moms who had had their tubes tied.
It far more likely to happen if the surgery was 10 or more years ago. Back then, they only cut the tubes, and tied a knot in the ends. Knots come undone.
More recently, they cut a portion of the tube out, then cauterize (burn) the ends to seal them. The likelihood of getting pregnant after this kind a procedure is much less.
