Have they mentioned preferences in certain items such as wine, candy, flowers, etc. I think a basket would be great filled with their favorites. You can embellish it inexpensively at the dollar store. Last Christmas I did just that. At the dollar store I got a pretty basket, a colorful doilie, and artificial Christmas greens. Then I put a bottle of wine, pretzels I decorated myself (in a dollar store container), a large candy bar, small jar of fancy jam. It made a beautiful presentation. I did two of these for friends, and they never guessed how really inexpensive it was. You can add fresh flowers just before presenting the basket. For someone who has everything, I think they would like it.

Buying could be an answer, but why not get really personal and offer a certificate (you can get a printable one online, I'm sure) for something only you can offer (no, I wasn't talking about the "X" rated type) just an offer to take care of details or chore(s) they hate or some special thing like an evening or day of company doing their choice of activity, an introduction to someone or something you know well -- use your imagination to come up with something no one else would even think to offer....

personally, I would create something for them. Usually these are the best presents to get. You can put together a music compilation, make them a card, write a song, a poem, make jewelry, or you can go out and buy them some ridiculous item that they would never purchase for themself. You don't even have to be that artistic to create something meaningful for a loved one.

well give him an experiance instead:
guys love mystery. well that and sex.. so start it off with a little bit of a mysterious feeling to it....
Send him an invitation - a card not email - that invites him to meet you at some upscale hotel on such a date and time. Then... do your homework!!! get a hotel that has like a honeymoon suite or someting that has like a hottub in the room.. then get there a few hours before him and prep!!!!!
have various assorted sexy outfits in the closet.. have the bed turned down... order dinner to be delivered soon after he arrives... have on some very sexy yet almost revealing outfit to wear while you are sharing dinner.. then.... do a nice slow strip tease for him and have your first round of "fun"... then take a dip into the hottub for a bit.. have some more "fun" in there... then get out and change into a different out fit and have more "fun"... order up dessert (some strawberrrys and chocolate) have it delivered... answer the door in your next outfit (don't cover up for the hotel staff) then invite him to have desert off your body...... I'm sure you will will find a couple more sessions of fun before the sun comes up.... wake up before him.. shower.. but on your next outfit and give him a nice wakup by climbing on top and riding him... then order breakfast while jumping into the shower with him.. then enjoy the nice breakfast together and recount the night before
If you really want to spice it up.. set up a camcorder on a trypod to capture all the moments!

Buy something that he wouldn't ordinarily think to buy for himself, but that he'd enjoy anyway. It should be something that shows you know what he likes. Some kind of artwork is great (if you know his tastes) or memorabilia from his favorite sports team, whatever. Even if he could buy it himself, the fact that *you* got it will give it added sentimental value. Everytime he looks at it or uses it, he'll think of you.

What about if you wrote them a really sweet poem and them bought a frame and framed it? Would that work? That is always really tough. It is hard when people go out and buy things all the time and there is nothing left to buy them. Or try http://www.thingsremembered.com

A professionally taken picture of yourself, a collage of pictures ,ticket stubs,etc of activities common to yourselves.Cook a special dinner, take them on a picnic,or some other outing they might like.
