Where can I download a form CMS-838 Medicare Credit Balance Report?

I would like to download a Credit balance reporting form and be able to fill in the required fields. ie. Pdf file that I can input into.

I need to be able to fill out the form - not just print out and type in. The CMS Medicare download site has the form but I am not able to input to the form.

You need to contact the main credit agencies. When you speak with them, get their address details and which department you should send a copy of your statement which states you have paid the debt. I believe credit companies are required to update your credit history every month, so a year with no update is unacceptable. They do this at a certain date each month, so if you contact them after the date that they update records, you may not see a change until the following month. Make sure you ask each company to send you a confirmation letter stating it's been corrected otherwise, if you apply for a loan or any type of credit, it will slow the process down.
