In most cases the transmission cooler which is located inside the radiator on vehicles with an automatic transmission can rupture and mix oil with the coolant and usually looks like a strawberry milk shake.
A bad head gasket will usually cause coolant in the oil not the other way around.

blown head gasket will not cause oil into your cooling system, instead it blow your coolant of the cap and cause engine overheating.
a crack on the block, means a coolant into your oil, oil will become milky and frothy.
two common source of oil into the coolant are the engine oil cooler and the water pump.
crack on the oil cooler tubes injects oil into the coolant,,this happens when the engine is running since oil pressure is higher than the coolant pressure.
bad water pump seal causes oil into the coolant, the seal separates the water from oil. once the seal is bad it will leak oil into the coolant side, specially when the weep hole in the pump is plugged. replace the pump to correct the problem.

busted water jacket in the head or blown head gasket between water jacket and cylinder. this will cause it to leak into the water line. and what goes into the cylinder comes out of the exhaust in the form of steam.
