Actually, this is easy, but it takes 2-3 seasons.
My best recommendation is find a horse farm nearby. They'll probably have something called "composted manure." (Hold on! It's not that bad.)
Composted manure does NOT smell. (Fresh does.) It'll add organic matter to the yard, help fill holes, and provide natural fertilizer to the yard. The best news - is that it's free! You just have to go get it. Often, the farmer will help you load it.
Use a shovel to put it where you need it - but make sure to cover the entire yard so it's an even green color. Use a garden rake (not a leaf rake) and flip it upside down so the tines point up. Run it back and forth across the piles to make it level. Don't put more than an inch down at a time.
If you have really bad holes, consider some topsoil to fill the holes, and re-seed or re-sod on top of the dirt.

If the holes are small, yes the grass will grow through it if you fill the holes with soft dirt like potting soil. or you can shave the high spots and put the sod in the low places. sow seed in the bare spots, they will fill quickly this time of year, keep it damp.
probaply what has happened is there were trees and the stumps are being consumed by bacteria and grubs. this is happening several feet below the ground. the soil is a living organism and it moves around a little.

You get a roller and fill it with water and hook it onto the back of the lawn mower and drive around the yard when the earth is soft. Like the day after a nice rain.

If you have plenty of time and experience doing it, go for it.
(hard labor, stress on back)
I strongly advice you to have professional landscaper to do the work for you.
You know for a fact they will get the job done right the 1st time.
The only thing is that you got to pay them, oh well right.
I found my landscaper @ home depot, they guys that hang on front of the parking lot, they don't speak English that good, but they will get the job done. Negotiate w/them, money is money, good luck....

Most big landscaping companies can do this for you. They haul in a truckload of dirt and dump it in your yard to level it. The grass is ok, it just grows right through it.
