It is part of the wrestlers personality, part their ideas, part writers. When they get that "almighty" visit with Vince or Jim Ross(who is the director of personel), they introduce their idea for a gimmick. If he likes the gimmick it's a go or if he likes the charachter he'll give them a thumbs up and the writers and talent scouts will help them develop their gimmick.

The gimmick of a wrestler is created by the wrestler and the creative department together. Depending how popular the wrestler is and how much influence he/she has in the WWE (for example), the wrestler might have more say in what character they play.

Newcomers and lower mid-card wrestlers have to obey the creative team more. However the bigger superstars (Triple H, Steve Austin, Shawn Michaels, The Rock, etc) have much more say in how they portray the characters they play.
