He has some health issues already so I wonder if it has anything to do with his diabeties (type 2), asthma and takes aspirin for thinning his blood.

What does he do for a living? Metallic odor usually means he is in the metal working business. Welding, foundry, machinist, etc. If so, what's happening is the metal his body picks up at work is sweating out. A couple of times a year, he needs to go to a hot spring for a purge.
