It depends on your work. The FMLA just insures that you have a job when you get back from maternity leave (or whatever medical leave it is). At my work, you get paid for it if you are full time. Part-timers do not unfortunatly.

Sometimes. If you are on disability, and they know that you are not coming back to work, they can count that time as your FMLA, or part of your FMLA.

Depends on your company.
FMLA is UNPAID leave.
But some companies will have paid short term disability or maternity leave that runs at the same time as your FMLA leave.

depends on your company and if you have any leave saved. FMLA just ensures you have a job to go back to when it is all said and done

yes or no....depends on the type of benefits your job offers and how long you are out...fmla doesn't promise pay - it promises that you will have a job of similar function and similar pay upon your return
