i am not sure where you are from, but here in louisiana anything that you sign your signature on your are entitled to a copy right then and there., or you can refuse to sign. even your time card,if you ask they have to give you a copy. when you are hired into a new job , the applications you sign agreeing to do the work and so forth,the companies policies, etc....IF ,upon request , they have to give you a copy. look up the dept.of labor in your state and either download or copy thier laws or call them and get the info you really need if it is a problem with your job tell them about it and then contact the EEOC-and file a complaint and as long as there is an investigation going on into thatcompany ,,that job CANNOT fire you or you can then sue them.

i was on workmans comp for a while and they always sent me and sent my job copies of everything so what ever they have you should also have and if now tell them you want a copy for your own personal records they have to they shouldn't have nothing to hide they are your records . just ask them if you can look at your file and get copies of what you dont have they should and if they refuse that means they are hiding something from you that isn't legal look in to it good luck .

this is considered part of your medical file, if you ask they by law are supposed to supply a copy of it to you.you have a right to any of your medical files
