I am going to japan on my own. But the legal age in Japan is 21.
So the general consulate of japan asked me to bring a parent's permission. I don't know where to write the date, name, address
and what should be on the paragraphs and all that. If you know how-to please help me. PLEEEEEAAAASSSSEEEEE.

Apart from the fact that this has nothing to do with Italy, I'm sure if you ask the Japanese consulate, they will give you a sample Parent Permission letter.....

Why don't you get on the phone to your parents and ask them to write the letter for you? I don't think the Japanese consulate will take kindly to a forged letter. Especially if they find it while you are in Japan.

Call the UN and ask to speak with a Japanese consulate. He/she should be able to help you (plus offer you advice on further things)

If you're going to Japan on your own, you're old enough to look up "business letter" in google and figure it out yourself!!
