i think the first should be the frontline japaneses. japaneses have been abruptly kidnapped in earlier decades and held in north korea against their will; some have been trained to be spies on return to japan. even those who have returned have learned to accepted their fate after decades of absence. .
another reason is the vulnerability of japan to the north korean missiles. north korea had demonstrated the capaiblities of their missiles by actual firing missiles over japan.while no effective counter against the missiles is in sight. it is hard to tell whether the patroitic missiles by american will do a good job for the japanese defence.
next is the americans. they are talking of curbing the north koreans on nuclear weapon test and have been helpless of the long-range missles capable of reaching any part of america. north koreans have long regarded the americans as hostile and has been reluctant to talk with the americans. without saying the talks by six parties reached an impasse a long time, despite
posturing by the six parties.
ironically the south koreans seem to be talking the most with the opoosite side. besides relatives across each other's border for reunion, food aid, joint venture, there are the many outstanding issues between the two korea.
reunification of the two koreans is only a dream now.
though china is closest to north korea, she is also worried of the instability to the region by north's belligerence (nuclear weapon and the long-range missiles). Poverty and natural disaster is the underlying cause of the north koreans, and there is a limit on what china can do about the current north's amibition.
there is not much to say about the russians.
last, countries near and around north korea will always feel the uncertainty of what the north will do next.

Japan is worried, still some old WWII grudges.
South Korea & US - still in a technically unfinised war.
China - Must choose if it is still a supporter of NK, pretty much a lose-lose choice for them whatever they do. Either a collapsed state next door if they cut off aid, or diplomatic crap from other countries for supporting NK.
Russia may get some fallout from a test.
If North Korea's govenment fell, South Korea might take it over peacefully, like East & West Germany merged.
