Sorry to spoil the fun but I've just found something on Google:

If removing the nail for medical reasons than maybe a numbing agent might by injected but with regards to pushing anything under the nail YES it hurts - got a wood splinter under the thumb nail last week which went the whole length of the nail
IT HURT A LOT!!!!.

Ah, but if you check further, you'll see that a method has been found for PAINLESS injections under fingernails!! Just need to wait 5 years or so.

Just for the record, yes it would be very painful. Inserting bamboo splinters under the nails was one of the torture methods employed by Japanese soldiers in the Far East during World War II. You will know how painful this is if you have ever got a splinter under your nail. Believe me it hurts! Lots!

Yes, yes, and yes, when I decided to clip the end of my finger off in a printing press, the plastic surgeon injected the locals into the end of my finger that was still available. And he was able to save the nail bed so the finger, yes it was that longest finger, has a nail now and 10 years later also has the nerves and feeling in the tip.

Highly unlikely after all the fingers have a vein running down the side. Only reason i can think of for inserting a needle there is to relieve pressure from a crushing injury eg black finger nail broken finger tip etc. Had it done once it doesn't hurt the relief from the built up pressure is bliss

I broke my toe once and had to have three local aneasthetic in my toe including one under the nail. Cried like a baby.
