First you need to go to court and file contempt charges. I would recommend you do it Pro Se.(without an attorney). Just to save money and it is a very easy proceedure. A court date will be set for a hearing. They will mail the information to your ex. He doesn't have to appear, he can send an attorney in his behalf. Usually the judge will hear the charges, then give the defendant (your ex or attorney) a chance to respond as to why he is not following a court order. Usually a judge will order all arrears to be caught up in 30 days or a body attachment will be issued an a warrant, and he can and will do jail time.
However, each case has different circumstances... for instance if he is in the military nothing will happen until his term of duty is over and then he will be given a reasonable time to catch it up.

Have your lawyer file on him for not following a court order. I doubt they'll go get him, but if he has assets, etc., here in the states, they could be frozen, etc., until it's all resolved. Good luck.

Depending on the extradition agreement between the US and the other country. He is going to have to be extradited back to the US for any charges that you want to file against him.
