proper condom usage leads to a 15% failure rate as a birth control, about the same percentage as 'pulling out'... I think it's on the box

No.....the only reason she would get pregnant would be if you used an old condom that has past its expiration date, if it had a hole in it or it was not put on correctly. It is so rare for the above to happen. If you are 100% positive that none of the above happened then you should not worry about it. She should not be pregnant.

everyone knows that a condom is NOT 100% safe. semon could have escaped out or the condom could have broken or split some how. Soooooo YES a woman can get pregnant even after using a condom.

yes, If the condom isnt effective and breaks or tears there is a chance semen can enter the womans vagina.
Just make sure you use the condom right, you should be fine!

Yes. Condoms are not always (but mostly) perfect.
Every now and again there can be holes.

definately!
Firstly these contraceptives arent 100% effective, they were introduced mianly to prevent diseases and stuff.

And today's women (Not all of them) have special tricks of getting pregnant, such as poking holes in the condom.

condoms and other forms of birth control are not 100% accurate.

you could be that percentage that it didn't work for. and don't forget they don't always prevent std's either.

yes it is possible because condoms are not hundred %sure.it leak or bust or better still it may slip during the course.
