Apparently the outside of my exhaust is well rusted and i believe it's also the same as the inside. What happens if you don't fix the exhaust for a long time?

Well eventually it will rust through the pipe and yes create more noise and fumes. But depending on were it rusts through you could actually cause damage to sensors or lose backpressure making your engine run less efficeintly and especially if it is a newer car will cause check engine light to come on.
