Because their head is too big to pull back the other way. Their ears get in the way and it just hurts to try to pull it out the way it slipped in because there's too much resistance.

I think the ears get in the way coming out of something. Or the hair. It's smooth and slides in then when you go to remove your head your ears and hair does something.....That is a really good question.

Your ears act like hooks, believe it or not.
Putting your head in, your ears get compressed to the side of your head whereas when you try and take it out, they get stuck.
Just like a fishing hook...

Usually they get too excited, and the blood starts rushing to their head (seriously), and they can't find the correct angle to get it out.
If they become calm, tilt their head down (as if looking towards their crotch), they should be able to get their head out.

It's easier and less painful to squish your ears back (when sticking your head between them) than to have them pulled forward (pulling your head out).

it is because of the way the head is shaped as well as the hole it is going in. same as an arrow is hard to pull out.
