I really like Old Navy & they always have racks of clearance items. I just got two pair of jeans for $16 a pair on sale. Only thing you have to watch is sizes.
Any other store I wear size 3/4 & the ones that I just bought @ Old Navy were size 0 so their sizes are way off, just try everything on first.

Everyone might laugh but for the most part Walmart has good quality clothing and a great selection for the prices you are looking for.

Old Navy has good quality clothing, but their tops seem to be made for extremely busty women and their jeans are pretty much made for women with no hips. If that's how you are though, you'll be fine.

A lot of Lands End's clothing meets your requirements. They have really good quality clothing, fast shipping, far better than what you will find at walmart or other cheap stores.
If you check their overstock section you might find some really good deals and their normal stuff is at the top of your pricerange. Its a place to keep an eye on.
