NO SANDALS OR OPEN TOED SHOES...
Other than that, nice, vintage-wash jeans with a button up long sleeved shirt is always great.

It all depends on what your plans are and where you are going.
Call me a hopeless romantic, but if you and your date are going to the theater to see a play (see first link) or non-rock/pop/rap/country concert (in other words, a concert where you would sit down and just enjoy the musical performance), and maybe go to a very nice restaurant that has linen table clothes, where the menus are Ala-carte. Then, you might want to let your date know to dress up, and then wear a suit and tie with leather shoes, which are polished. If you drive, it would also be a good idea if you open the car door for her, because it's the kind of thing that gentlemen do. I have been with my wife for over 22 years and the romance is still alive in our relationship.
If you don't have a suit, wear a nice pair of slacks and a sports coat with a tie. If you ask me, every man should at least have this in their wardrobe if they do not have a suit. I have a suit, a sports jacket, two pair of dress slacks, white dress shirts, and a big collection of ties.
Anyway, if you are going someplace that is not as fancy, then just dress appropriately for the place you are going to. Just make sure your clothes are clean and not full of holes. I wouldn't wear work clothes either, especially if you are a mechanic, painter or work in another profession that has a tendency to ruin clothes.
Unless it's approiate to wear t-shirts as outer wear to where you are going, I would aviod wearing them on a date. But, if you are and your date are going to a paint ball place, then it would make perfect sense to dress down in old clothes.

If you look good in jeans, wear them. Choose a nice shirt, if you have muscles, show them off in a nice form fitting shirt. Don't forget the cologne, very important. If you smell good, the clothes won't make a bit of difference. Oh, and breath mints. No one likes bad breath.

Specifically....and this is just my preference.
For the Fall Season:
Dark denim jeans, nice thick wool turtle neck sweater, leather jacket/coat that goes to mid-thigh, black leather boots or complimenting shoe/loafer.
Great cologne, a wonderful smile, and bright eyes.

Jeans, a band t-shirt, and a suit jacket or button-up shirt overtop. Basically a very pulled-together but casual but dressy look.. I know that made no sense, haha. I'd say a suit, but sometimes they just make people look stuffy.

An expensive cologne - but not too much of it.
