I know it was called "I am the Doctor" if that is any help. It was a spoken word poem Pertwee wrote himself with the Ron Grainer theme music.

He did one as Worzel but I don't know of one as Dr Who. Mankind did a song called Dr Who, and the Timelords did Doctorin' the House which had the Dr Who theme.
