A method you can do on yourself, is to apply cross-friction to the scar. With your thumb or fingers (depending on size of scar), briskly rub back and forth across the scar tissue. This helps slightly with appearance, and also breaks up the scar tissue around it to prevent dysfunction.

If they are old scars, not much you can do at home. You would have to see a plastic surgeon, they can usually minimize them.

Try Maderma - it's availabel OTC at your local pharmacy. it really helped with my wife's scar after her surgery.
