Most group it as people who get a sex change operation. Truthfully, it's more of someone who doesn't feel that their gender role (which is assigned by what genitalia you have) is correct or is incompletely. Transexuals/Crossdressers can be put in this group, but not always; some straight men are transexuals, but are more than happy as a man who just wears women's clothing and make-up.
I would say a transgender is someone who simply wants to be the opposite sex for personal reasons. *shrug* That's MY definition though, you might want to do some research into it (Wikipedia is always good) and come up with your own definition.

To be transgendered is simply that the birth sex you were assigned due to genetalia, you feel to either be false or incomplete as a definition. So, trans-sexual people could be defined as a sub-group of transgendered people. However not all trans-gendered people are trans-sexuals!

It means that you are really mixed up and so afraid to be who you are that you are willing to have disfiguring surgery to be someone you're not. It also means you are a pervert!
