Simple solution:
Wash the clothes in hot water with good quality detergent. As soon as the load is done, place them in the Dryer until they are completely dry. Immediately remove them from Dryer and fold.

Sometimes clothes will sit in a washing machine for a period of time and then dried. Once you wear them and sweat slightly, the damp mildew smell presents itself. I found the above process works everytime.

Good Luck :)

Use the Gain detergent with the Gain fabric softener. It's the one with the fruity smell. That works really good or Tide powder form makes stuff smell really good. I have found it has a better more powerful smell than the liquid Tide.

Wash clothes in 1/2 cup fabreese. Then rewash as usual. I used this method with some clothes I bought from a garage sale. Let me know how this works for you.

Rewash the clothes and add a cup of baking soda to the wash cycle. Works every time.

Wow, vinegar didn't work? Maybe you didn't use enough. I use a couple of cups per load. I've also heard rubbing alcohol and tried it, but didn't seem to work as well as the vinegar.

ADD: I just remembered my mom told me the best way to get rid of that is to wash them and dry them and they right away re-wash and re-dry them. I'd go with the vinegar though.....

Sometimes that happens to me if I leave them in the washer too long, or leave them in a basket or let them hang dry in the house, I just rewash them, and pour a little extra detergent in there and it comes right out.
