Luke Appling , White Sox (1930 to 1950), is said to hold that distinction. The reason I say distinction is because there isn't really a stat. kept on this. Supposedly, he had pitch counts that ranged from 19 to 24. He once battled Bob Feller with four fouls, one ball, six fouls, one ball, fourteen fouls, and then two more balls for a walk.
Pat Listach once took 17 pitches in an at bat, when he was with the Milwaukee Brewers in 1992.

Ricky Gutierrez holds the documented record with a 20-pitch at bat on June 26, 1998. He was with the Astros and was facing Bartolo Colon and the Indians. He started off down 0-2 and then ultimately struck out on the 20th pitch (he had worked the count full). His at bat accounted for 18% of all the pitches Colon threw that day.
Alex Cora's at bat was an 18 pitch at bat that ended in a home run (Wiki, MLB.com). It is currently the third longest.

Well, the most pitches without fouling off a ball is 6...so it would have to be the same at bat.
The problem with your question is that this stat was probably never even kept (it's not really a stat) back in the 30's, 40's, 50's, etc...you will probably never get a true honest answer to this question, because of that.
I have seen 14 pitch at bats in recent times...but I am sure somewhere in the 20's, some guy fouled off 20 pitches or something...

I do not know.
But I remember Alex Cora on the LA Dodgers a few years ago had 21 pitches and hit a home run on the 22nd pitch and that was the record for the most pitches in one at bat to end in a home run.

The guy above me, I believe, is correct when he mentions Luke Appling. Appling supposedly once purposely fouled off 14 consecutive pitches in an attempt to get even with an owner who hadn't fulfilled one of Appling's requests.
