Nah...I prefer to live dangerously. I just make sure the restaurant isn't near an animal hospital.

Yes I definitely ask before I eat foods that I'm not familiar with, even if they aren't foreign, because I have to know what I'm eating before I eat it, and what are the ingredients in it.

I'll usually dive in to ethnic cuisine without hesitation. Foreign objects are a different matter. If I find anything that I think shouldn't be in there, I'll ask.

Sometimes. Not long ago, I was in a Thai food place and I had to ask the lady, "Well, what's IN that one?", pointing to the menu. She said, "Oh, that's just noodles and beef, and stuff. No body parts."....body parts?
