A mutant would be a person who had a change in their DNA... OK, so that would include almost everyone! ALmost everyone has some tiny change in their DNA from what their parents had. Usually it is some very tiny change that doesnt cause any harm, but once in a while it is a more important change that really causes significant damage to the child. So you are probably a mutant already.
OK, there are quite a few things that you could do to make yourself a mutant, but that would be a REALLY bad idea. In humans, mutations are pretty often called "cancer"! Most of the mutations that could happen to you would be a damage to your normal DNA. Hardly any mutation could happen that would make you improved, and definitely you would not be able to get any mutation that would give you super powers -- that's jus tin comic books!
Trust me, you do NOT want to be a mutant!

you watch too many movies ;-)
this being said, maybe you didn't know it, but we all have small mutations, typically caused by errors in the transmission of some genes because of, say, gamma rays from space, or some chemical contamination (pollution).
this was always there, we're geared to resist it up to a point, and from time to time this results in someone developing a favourable characteristic.
i personally have a very very tiny tube in the lobe of my left ear, it serves no purpose whatsoever (and it even got infected once). This is not part of the normal program for human beings, so that makes me a mutant, of sorts. Most people you know will have some (very) small such thing, most often less easily detectable than mine, but still.

Sure thing! Just break yourself down to a subatomic level, and then transform your DNA outside of the normal human range.
Piece of cake.

most mutations, if they have an effect, either lead to cancer or death in some other way.
