An auto parts store will carry an adhesive made specifically for the rear view mirror. Just follow the directions, and it works. It usually costs around three dollars.

Go to an auto parts place like Napa. There should be several different replacement mirror options. The base piece, the one glued to the window, is a standard piece, and it's size is the same in almost all cars and trucks, so finding a mirror to fit is no problem. However, if you have any sort of electronic switches, bells or whistles on the broken mirror, you will have to go to the dealer to get an exact replacement.

Take it to any reputable auto glass repair shop.
They will put it on for free or for very little money.
