Hewlett-Packard and their 2004-2006 board leak investigation process - which is still unfolding.

They broke laws and damaged their reputation, and may have taken out their highly-esteemed CEO, or at the very least exposed themselves to tons of fiduciary-duty lawsuits.

Precisely what went wrong in their decision-making is still unfolding, in a very public way.

Coca Cola and the introduction of coca cola new formula back in late 80's.Coke did market research that confirmed the fact that the "new formula" was better. Coke launched the product. Cost them million. Poor sale. Back to the drawing board, they realised that there are 3 things that US people were not ready to see change: Apple Pie, Uncle Sam and Coca Cola. The yre-launched Coca Cola Classic to try to recapture their market share. Pepsi and others made a big move at that time.
Not sure you will find anything on the web though... companies do not like to advertise their mistake.
You may find more example at the library in good old fashion books! talk to a good librarian.

Enron: The rise and fall of a business giant. It's a twisting tragic weave of white collar crime that took down some people thought to be untouchable.

Enron rose from humble beginnings to become what APPEARED to be a mega solid global industrial / finance giant. But when the internal company profits began to decay, they quietly stole $$$$ from their own employee benefits fund--which got uncovered. By that time, Enron crashed---hard--and left many employees wiped clean of their life savings and too old to start over.
