To keep someone on their toes means to keep them guessing, keep them anxious.

When u "keep a person on their toes" it means that you are not boring and you can keep that person wanting to keep coming back for more. It means that you are offering something extra to the relationship. If you are in a serious relationship, sometimes it helps to do this with your girlfriend/boyfriend because they recognize that you are helping them with life situations. For example, if your boyfriend/girlfriend smokes all the time but wants to quit and doesn't know how to kick the habit and you help them by not buying the cigarettes as often or help them by getting them on the patch or keep up with how many cigarettes they smoked that day, you are keeping them "on their toes" and helping them to be a better person. Or say if u meet someone and they say they are looking for someone that knows how to party and have fun and not be boring and bland and you go out with them and they say they had alot of fun with you because you keep the conversation going or you kept them moving and dancing all night, that means you are keeping them "on their toes" and in high spirits.

It means to motivate a person to be cautious, alert and on top of things. For example, a young child always keeps "Mommy" on her toes because she'd better pay attention at all times.
