Seeing someone's body contort in a totally unnatural position like when I accidentally saw the trailer for the latest Exocist movie. She was walking down the stairs like a spider. The creeps me out the most.
Oh, and flying cockroaches.

It is probably walking at night through an area that may or may not be dangerous. Alternatively, nails screeching on a blackboard is another.

nails on a chalkboard, finger nail files, styrafoam, cardboard boxes, sand, balloons (regular, not the foil ones), chalk, ITCHY SWEATERS. I have OCD, and a symptom of it is complete intolerance of certain sounds/feelings.
