The first thing he should do is visit a therapist that is trained in gender identity issues. Let them take it from there. Any further advise should come from a licensed professional.

I wish him luck!

It's a very complicated issue. There is no proven explanation for this phenomena. Some say it is biological, some say that it is psychological.

A trained therapist may help him sort out his feelings. Some men chose to become women through surgery. Some go the less expensive route of making themselves appear to be women and living as females. Only your friend can really know what is right for him. Support him and remind him that this is not an issue of morality. Tell him not to let any holy book-thumping wackaloons judge him. They are no experts on mental health.

If he is serious, he should seek counseling from a counselor familiar with gender issues. He may have a genuine problem which can be helped. Maybe he is transgendered, and he can learn different ways of coping with that. Remember, not all transgendered individuals undergo gender reassignment surgery, but all who live well adjusted lives get some help! If your friend doesn't live in a larger city, try looking at a big city near you to find the counselor. Your web browser is your friend.

I know this may sound wierd...but I use to date a guy that had gender dysphoria. He thinks he was born a guy but was suppose to be a girl. He thinks that inside he is really a female and things just got messed up when he was born. He acted really girly and truly believed he was a girl on the inside. He wasn't gay though...he described himself as more of a lesbian b/c he was only attracted to girls. I couldn't deal with this so I left him. I just needed someone who was a man...inside and out. I would be there for your friend. As my ex said ...he didn't CHOOSE to be this way...it's not like he wants to be confused about himself and his sexual identity. It's an actual disorder, so understand that it is not his fault he's having these problems. Just tell him he should seek counselling. There are plenty of therapists out there for situations like this. If thereapy is out of the question, just be there for him but let him know that he WILL have difficulties in life with this, just like any other disorder. If he chooses to have surgery make sure he really thinks it through. Hope this helps.

Do not think that you or any body without professional knowledge or training should give advise on this subject. What you can give him is your support, let him know that it makes no difference to you. That you will remain there friend regardless of there sex or sexuality. In this you can help him through this, just by letting him know that you will support him whatever his decision will be

Tell him to see a gender counselor. If he's serious and totally wants to transition he has to have 2 letters anyway unless he goes to Thailand. But regardless he'd have to have a referral for hrt so he might as well start with a counselor. They can help him sort out what's in his mind.

There is always surgery. Let him be who he wants to be and support him if he wants surgery or something like that.
