The form that you want for an atomic notation is: (ignore the .....)

A
....X
Z

where X = symbol of element
A = Mass Number (# of protons)
Z = Atomic Number (#protons + # of neutrons)
p = number of protons = A
n = number of neutrons = A-Z

You just have to figure out what "A" and "Z" is and put it in that form. The first one would be:

6
....C
12

By asking for atomic notation he/she is only asking for you to put the element's symbol alongside its atomic number and atomic mass.
I think your teacher is trying to push the idea of isotopes.
