Call your local federal office and request a form. Or go to the office and pick one up. They may also have them online. Then you fill in the form and you can fax it to them if you have a fax machine and they will fax you back with the number.

If you need to find one for an existing business, check with the agency that issues business licenses in your area. If you need one from a former employer, it is found on your W-2. If you need to apply for one, you will receive one when you get a business license.
