How would you handle recommendations for a job opening especially if it was from a fellow employee? Giving it special treatment would create a biased decision. No matter if it was from the VP or another manager, i believe applicants should be handled the same way regardless of their source. Right?

Right they should be
How I handle of them is I'll definetly give them the test or interview, but they have to pass it the same way as all the other applicants. And the person who recommend them do not know what the interview questions are or what the test is. So there is no help that way.

Also even if they do pass but they are at the bottom and i want 6 and they aren't part of the top six, i don't give them the job...
They may be a friend or relative, but its about the best for the company in the end.

A recommendation is just that. Just because someone is recommended doesn't mean they should automatically get the job. They should have to show they are capable of the job just like any other applicant.

Yes, applicants should be handled equally, it's the law. And hopefully the most qualified candidate gets the job.
