it depends on the doctor / clinic - could be free up to $300
though in the USA if you do not get reimbursed you can claim it as a tax write off

If the job requires a physical examination they should pay for it, and give you a list of physicians that they suggest you use because there is a contract with those physicians on the list.

If you had your interview and they asked you to go for a physical they usually give you the name of a place to go because they pay for it.
