The nicotine test is through your urine and can take several days to get out of your system. Doctors can also do a breath test and this measures the level of Carbon Monoxide in your body. (its higher for smokers). The key for you is to quit using as I am sure your doctor told you that you will not heal as well when you smoke, thus the reason for not smoking after back surgery.

i took one when i went to a stop smoking group, it's like a Breathalyzer test. You blow into a tube and it comes up with a reading. Not sure how long nicotine stays in your system, but i hadn't had a cigarette for a day and the reading was low, so at a guess i would say a couple of days.

they can also take a cotinine saliva swipe. the make you suck a swab and send it off for the results. You need to stop at least 1 week before test.
