Poverty, lack of medical care, lack of people administering medical care, lack of clean water, lack of education regarding HIV, lack of prevention (like condoms etc), i heard that the Minister of Health of South Africa suggested that beet and garlic is a better medical remedy as opposed to retro-viral drugs, that doesn't inspire much confidence. Take your pick dude
