I've seen this proverb been cited many times. I guess it's simply because in four words you can say a lot. Most of the times to earn something you just have to work hard for it. Being so, it means that it's worth to work for something in order to achieve your goals. Why do I think you already knew this???

It is justification for spending time doing unpleasant things. It also leads to the thinking that gain requires pain, which is not always the case. So I think of it as sloppy thinking.

this adage refers to the thought that, in many athetic related activities, you must work hard to get better. for example, if you want to be a top flight runner, you must run and run and run. this "pain" comes from sore muscles, the "gain" is that you build endurance to run farther and faster.
it also can be applied to other areas of life, both figuratively and literally

If you don't feel the pain (of exercise), then you aren't going to gain anything from exercising.

Most people aren't trying to gain weight they are trying to gain fitness and weightloss.

its like the proverb over the entrance to concentration camps -- arbeit macht frei -- in mortal life people like to think that sweat and toil will get you somewhere other than just working your butt off to make those above you richer
