Most leave requests these days are done via forms but if I had to write one as a letter it would probably look something like this:

Dear Mr./Mrs./Ms. :

I would like to request one day of personal leave on . Before I leave I will let my coworkers know what I've been working on so that they can cover for me in an emergency. Please let me know if there would be a problem with me taking this day off.

Sincerely,

To be honest, you use a lot of inappropriate words, and the majority of it is very awkwardly phrased. It sounds like english is not your first language (is it?), and you say a few kiddish things (e.g. "i will never make the same mistake again"). Bottom line is that a small difference in your SAT score was not the reason they rejected you, this is not going to make any difference most likely. Also, it sounds like you were careless, applied later than you had to, and by the time you realized you turned in the wrong test score it was too late. This shows how incapable you are of following simple instructions. You admitted this in your letter, but it is too late. The entire first paragraph (except at the end where you talk about SAT scores) is complete bs and fluff. You talk about how much you want to go to UCLA, yet you have no reasons. UCLA is only your dream school because of some nonspecific emotions you have for it. If you had actual reasons to go there, you should say so. No one really believes something like "UCLA is my dream school," every prospective freshmen says that, and almost none of them know why it is. "admissions personnel" is not a proper greeting, it sounds awkward. This may sound strange but opening with "To Whom It May Concern" with all first letters capitalized is actually proper, or even Dear Sir/Madam, or something else. You make the admissions committee sound like some security task force. Just apply again next term. It is annoying to wait, but this is how things turned out for you. If it really is your dream school, then you can wait. If it is not, then you will just go somewhere else. See how much you really want this lol. UCLA is hard to get into, you might not even make it in next time, so you should be realistic. By the way, if there is not a formal process of appeal available, this is a little silly to be sending. But, if your mind just cannot rest unless you try as hard as you can, then go ahead.
