Is this a problem with the linebackers, safeties, defensive scheme, or something else?

I'd have to say more problems with the linebackers than the safties for receptions made by tightends.
The linebackers need to jam the tightends at the line of scrimmage and/or cover the short zones where most tightend receptions are made.
Safeties only get involved in tightend coverage on goal line coverage, deep zone coverage and secondary coverage to the linebackers.

the answer, believe it or not, usually is found in the safety posistion. often in defensive schemes, the safety is used to cover the te, as i'm sure you know. but plays like tight end seams and other plays where recievers go on short routes and TE's go on long ones can sometimes make a safey bite on the wrong play, and leave a streaking tight end wide open. Also, if a safey bites on a play action fake, which they do more often than you'd expect, a tight end can get open over the middle for a long touchdown catch.

In the middle of the field , most TE route's are in the middle with all of the LBs and some Safties.

generally linbackers. but it depends on the play, if there in the redzone, if the D is in man or zone. that kind of thing.
