Call the college. You will have to sign a release form stating that you give them permission to give your records to the other college.

you call the medical establishment and request a copy of your medical records. they will send you a medical release form that you need to fill out and send back in. they will then release a copy of your records directly to you. you are entitled by law to receive 1 copy of your records from them. future copies will be of charge
